Based on a comprehensive, in-home assessment, a Minuteman Care Manager creates a personalized plan to meet your needs, preferences, and available resources. Your plan may encompass a range of services, including:

Our Care Managers are expert problem-solvers who understand the challenges of daily living that many older adults, people with disabilities, and their family caregivers face. They have the expertise—and the compassion—to help you understand your options, secure services, or find an appropriate living situation.
