Service Provider Contracts

Thank You for Your Interest in Becoming a Service Provider.

Minuteman Senior Services contracts with service providers who work directly with our consumers. The Executive Office of Elder Affairs determines the types of services that we can offer. The Executive Office of Elder Affairs is responsible for approving agencies interested in providing Homemaker/Personal Care Services. For more information, visit their website at 800ageinfo.com.

The contract application process has closed for FY2017. The next procurement/RFP will be in 2019. You can view the forms below for informational purposes only.

If you are interested in applying for one or more Non-Homemaker service contract, you must:

  • Read the Bidders Notice and all background documents including the Service Description for each service(s) you plan on bidding on;

  • Complete and sign the Minuteman Senior Services Standards of Service and the Administrative Overview;

  • For each individual service being bid on, complete the Service Specific Application and a Minuteman Senior Services Rate Sheet. Please remember, if you are bidding to provide multiple services, then you must submit multiple Rate Sheets - one for each service;

  • Lastly when packaging the application please include the following documents in the Response: Minuteman Senior Services Standards of Service (signed), Administrative Overview (signed), Service Specific Application(s) (signed), Rate Sheet(s) (signed), Certificate of Insurance, IRS Form W-9, SOMWBA Certification (as applicable).
